Policy Summary
Cranial CT (with or without IV contrast) is covered when there is clinical evidence of an intracranial disorder or an established intracranial disease. Contrast-enhanced CT is specifically reasonable and necessary for cerebral perfusion assessment (e.g., CVA), lesion characterization, detecting blood–brain barrier defects or neovascularity, and for brain staging in known lung, breast, or lymphomas likely to metastasize to the brain; contrast risks should be considered and documented.
