Policy Summary
Medicare covers serum homocysteine testing to confirm vitamin B12 or folate deficiency when initial B12/folate test results are inconclusive or discordant with clinical findings. Homocysteine testing is also covered for one-time lifetime risk stratification only in patients with known vascular disease or vascular risk factors (abnormal lipids, hypertension, or diabetes); testing for screening in patients without vascular disease or for evaluation of treatment of hyperhomocysteinemia (including in osteoporosis or primary fracture prevention) is not covered.
