SCODI (including AS-OCT and posterior-segment OCT) is covered when used to evaluate and monitor specific anterior segment conditions (narrow-angle and mixed mechanism glaucoma when gonioscopy is not possible) and posterior segment conditions (glaucoma diagnosis/monitoring, optic nerve disorders, retinal disease, and drug-related retinal toxicity). SCODI is not covered for screening or absent a documented indication; other uses are investigational. Test frequency expectations are specified (e.g., AS-OCT ≤2/year for angle-closure diagnoses; SCODI ≤1/yr for glaucoma suspect; SCODI ≤2/yr for progressive optic neuropathies; retinal SCODI generally ≤1/month for aggressive monitoring) and testing must be supported by documentation and an interpretation report.
