Vestibular Function Testing

Vestibular function testing is covered when clinically necessary to determine appropriate medical or surgical treatment of vestibular system disorders and when testing results are used to manage the patient's specific problem. Coverage requires a prior clinical evaluation (history, physical exam, medication review), a written order and treating relationship with a physician, appropriate documentation of interpretation and plan of care, and use of qualified personnel (physician or state-licensed audiologist); nonspecific dizziness alone and testing solely for hearing-aid decisions are excluded.
