Policy Summary
One-day acute inpatient admissions for chest pain are covered when the medical record documents a clinician's ACC/AHA-guided synthesis of history, physical exam, 12-lead ECG, initial (and serial when applicable) cardiac biomarkers, and observed changes or treatment response that justify inpatient management. Coverage is at risk of denial when key diagnostic elements or the cognitive rationale are not clearly transcribed; hospitals are encouraged to adopt ACC/AHA workflows and fully document the decision-making process to reduce improper payments and denials.
