Policy Summary
Medicare hospice coverage for beneficiaries with end stage renal disease (ESRD) is appropriate when ESRD and any comorbid or secondary conditions produce structural/functional impairments and activity limitations resulting in a prognosis of six months or less if the illness runs its normal course. Documentation must include sufficient clinical information demonstrating the impairments, activity limitations, environmental factors, and the impact of comorbid/secondary conditions to support certification and a beneficiary-centered plan of care.
