This LCD covers skilled nursing teaching and training in the home health setting for Medicare beneficiaries with Alzheimer's disease and behavioral disturbances when services are based on the patient's medical condition and individualized assessment. Documentation must fully characterize each behavior and justify the care plan, interventions, and the number of training visits as reasonable and necessary; services are not covered if caregivers cannot or will not be trained after a reasonable period.
