Policy Summary
Medicare covers skilled speech-language pathology services that are restorative or, when skilled care is necessary, maintenance-oriented to improve, restore, or preserve cognitive, communication, and swallowing functions following congenital or acquired conditions. Coverage requires individualized assessment, a documented plan of care with measurable goals, periodic re-evaluation as clinically indicated, and documentation that services meet CMS skilled-care criteria; non-skilled routine maintenance is not covered.
