Home health physical therapy is covered when skilled PT services are required to improve, restore, or maintain functional abilities, with coverage contingent on documented medical necessity, objective measurements, and a clear plan of care. Specific modalities and procedures (splint fabrication, therapeutic exercise, gait training, manual therapy, ultrasound, vasopneumatic devices, orthotic/prosthetic training, wheelchair training, etc.) are covered for defined clinical indications, while non-skilled or heat-only modalities alone, TENS for chronic low back pain, work-specific vocational training, and routine post-education lymphedema pump use are limited or excluded.
