Policy Summary
Hospice eligibility for beneficiaries with Alzheimer’s disease requires a documented FAST (Reisberg) stage of ≥7 plus documentation of comorbid and/or secondary conditions that produce structural/functional impairments and activity limitations such that, in combination with FAST ≥7, most patients would have a prognosis of ≤6 months. Documentation must include the FAST stage, the specific impairments/activity limitations for AD and each comorbid/secondary condition, and a clinical rationale that supports a ≤6-month prognosis and selection of palliative care.
