Electrodiagnostic testing (NCS and EMG) is covered when performed by appropriately trained/credentialed providers and when clinical history and examination document the need; both EMG and NCS are generally required for diagnosing peripheral nervous system disorders. Limitations include single reimbursement per nerve (or per limb for preconfigured electrode tests), noncoverage of routine screening or monitoring for diabetic or ESRD-related polyneuropathy, exclusion of certain device-based or non–real-time testing modalities, and restricted use of automated devices (eg, NC-stat®) only for carpal tunnel syndrome with high pre-test probability (one service per arm).
