Medicare covers measurement of 25-hydroxyvitamin D (25OHD) for a defined set of medical conditions (e.g., CKD stage III+, osteoporosis, rickets, malabsorption, medication-induced deficiency, granulomatous disease, obesity, post-gastric bypass) and for monitoring vitamin D replacement when medically necessary. Measurement of 1,25-dihydroxyvitamin D (calcitriol) is generally reserved for rare endocrine disorders, renal osteodystrophy, certain rickets, or unexplained hypercalcemia (including sarcoidosis). Testing is not covered for routine screening; only one total 25OHD assay is reimbursable per patient per day, methods must be validated, and clinical justification must be documented in the medical record.
