Policy Summary
Endoscopic endoluminal treatments for GERD such as Stretta, EndoCinch, and Plicator are non-covered due to insufficient evidence. Transoral Incisionless Fundoplication (TIF, e.g., EsophyX) is covered when performed by a well-trained surgeon for chronic symptomatic GERD (>6 months) that is not fully responsive to PPI therapy, documented by GERD-HRQL criteria (>=20 off PPIs for 14 days or >=10 point difference on vs off therapy); repeat TIF and other endoscopic interventions remain investigational and are not reimbursable.
