Nerve conduction studies (NCS) and needle electromyography (NEMG) performed and interpreted together, on site and in real time, are covered when used to evaluate focal and generalized peripheral and selected central neuromuscular disorders (e.g., plexopathy, focal neuropathies, polyneuropathy, radiculopathy, myopathy, motor neuron and neuromuscular junction disorders). Limited exceptions allow NCS-only or NEMG-only testing (e.g., anticoagulated patients, lymphedema, carpal/tarsal tunnel, injection guidance, pelvic floor or laryngeal evaluation). Coverage excludes delayed/off-site interpretation, non-real-time portable devices, certain investigational or non-physiologic tests (QST, SEMG, macro-EMG), tests for asymptomatic diabetic polyneuropathy for monitoring only, and studies exceeding AMA CPT Appendix J reasonable maxima unless redetermined; frequency limits per diagnosis per provider are specified (generally 2–3 studies per 12 months).
