Neurophysiology evoked potentials (SEPs, BAEPs, VEPs) are covered when targeted to specific clinical indications: SEPs for suspected central somatosensory pathway lesions (e.g., spinal cord trauma, MS, degenerative myelopathies) with supporting history/exam and appropriate diagnostic testing; BAEPs for asymmetric hearing loss, tinnitus, sudden hearing loss, tumors, ototoxic monitoring, cochlear implant pre/postoperative evaluation; VEPs for evaluation of optic nerve disorders and to support MS diagnosis when clinical criteria are inconclusive. SEP testing for routine peripheral neuropathy diagnosis is not covered because nerve conduction studies are generally sufficient; all testing must be performed and interpreted by qualified physicians and documented with relevant clinical history, exam findings, and supporting diagnostic test results.
