Vestibular and diagnostic audiologic testing is covered when performed to determine appropriate medical or surgical treatment for auditory, balance, or neural disorders and when furnished by qualified providers within required settings and supervision rules. Specific coverage includes ototoxicity monitoring for >14-day aminoglycoside therapy, implant-related testing and programming, and physician-referred audiologist evaluations; exclusions include screening for hearing aids (except BAHA evaluation), home-based testing, certain hospital-based technical components, tests without physician referral, and computer-administered non-skilled tests.
