Nerve conduction studies (NCS) and electromyography (EMG) are generally covered when performed by appropriately trained/credentialed providers in labs meeting safety standards and when clinical history and neurologic exam document the need; EMG is usually performed with NCS unless a documented contraindication exists. NCS/EMG are indicated for focal/compressive neuropathies, traumatic nerve lesions, generalized neuropathies, radiculopathies, myopathies, plexopathies, and neuromuscular junction disorders (with single-fiber EMG for myasthenia gravis); screening for diabetic or ESRD polyneuropathy and sNCT testing are not covered. Providers should follow the AANEM model for reasonable study counts, document pre-test probability for NCS-only carpal tunnel evaluation, and limit EMG use for botulinum localization to one study per anatomic injection site.
