Monitored anesthesia care (MAC) is covered when provided by qualified, state‑licensed anesthesia personnel who are continuously present and when active monitoring (oxygenation, ventilation, circulation, temperature) is required. MAC may be separately payable for procedures that usually do not require anesthesia or for surgeon‑performed procedures only when significant, active conditions or risks—documented in the medical record—justify anesthesia personnel; routine, stable, treated conditions alone do not justify MAC and services included in the global surgical fee are not separately billable.
