Nerve Conduction Studies and Electromyography

Nerve conduction studies (NCSs) and needle electromyography (NEMG) are covered when performed and interpreted together on site and in real time for a range of focal and generalized neuromuscular disorders (plexopathy, focal neuropathies, polyneuropathy, radiculopathy, myopathy, motor neuron disease, neuromuscular junction disorders, traumatic nerve lesions, and specified symptom presentations). Limited exceptions permit NCS-only or NEMG-only testing for anticoagulated patients, significant lymphedema, carpal/tarsal tunnel evaluations, botulinum injection guidance, laryngeal neuropathy, certain pelvic floor/urogenital indications, and SFEMG for myasthenic conditions; delayed interpretation, certain device-based automated tests, QST, SEMG/macro-EMG, and other specified uses are not covered. Frequency is generally limited to two studies per diagnosis per provider per 12 months (three for motor neuron disease and plexopathy) with exceptions for multiple providers or multiple diagnoses, and documentation must include clinical history, exam, justification, and comparison for repeat testing.
