Diagnostic abdominal aortography and renal angiography are covered when used to evaluate acute traumatic abdominal injury, aneurysm/vascular abnormalities, occlusive disease (including intestinal ischemia), acute GI hemorrhage, congenital anomalies, pre-operative/interventional planning, and specific renal indications (severe or resistant hypertension, progressive renal insufficiency, renal neoplasm, hematuria of unknown cause, abnormal nuclear kidney imaging, and renal arterial defects). Coverage for concurrent renal or lower extremity angiography during other interventional procedures requires meeting stand-alone indications plus documented reasons (no prior adequate catheter study, changed clinical status, inadequate visualization, or intra-procedure change); repeat procedures and reporting are allowed only with clear medical necessity and documentation. Relative contraindications (e.g., severe hypertension, coagulopathy, contrast allergy, renal insufficiency) may restrict coverage, mobile units and unlisted locations are non-covered, and required documentation includes pre-procedure assessment, prior imaging/test results, renal function/eGFR, and adherence to Billing and Coding A56682.
