Coronary CT angiography (CCTA) is covered as a noninvasive alternative to invasive angiography or stress testing for symptomatic patients or those with unclear or conflicting noninvasive test results, for pre-procedural and surgical planning (including congenital anomalies and mapping for ablation/pacemaker placement), for CABG graft patency assessment, for ED evaluation of acute chest pain, and to evaluate ischemic etiology of new-onset heart failure. Coverage is limited to modern MDCT systems that acquire ≤1 mm slices and have ≥64 slices per rotation; CCTA is not covered for screening, for isolated calcium scoring, for atrial fibrillation/flutter without planned ablation, or when it will not provide additional management-altering information. Documentation must show the treating provider ordered the study, the study will be used in management, and appropriate supervision and beta-blocker administration (if used) are part of the test.
