Autonomic function testing is covered when used diagnostically to evaluate signs or symptoms of autonomic dysfunction—including autonomic or axonal neuropathy, syncope variants, POTS, and small fiber neuropathy—and to assess severity, distribution, or response to treatment when results will inform management. Screening asymptomatic patients, testing solely for disease monitoring in diabetes/hepatic/renal disease, tests not used for clinical decision-making, use of investigational devices for screening, insufficient equipment, and tests performed by inadequately trained physicians are not covered; documentation of symptoms, exclusion of common causes, equipment specifications, and provider credentials may be required.
