This LCD covers wound care for chronic, non-healing, or complicated wounds (e.g., surgical wounds healing by secondary intention, infected or necrotic wounds, wounds with biofilm, or wounds with complicating metabolic/vascular factors) and specifies indications for active debridement, NPWT, and MIST therapy when standard care has failed. Coverage requires documented evidence of wound improvement, appropriate adjunctive measures (pressure off-loading, infection control, vascular/metabolic/nutritional management), and adherence to stated contraindications and frequency expectations (e.g., MIST 2–3x/week with improvement by ~6 treatments); investigational methods are not covered and some routine dressing changes are not separately billable.
