Policy Summary
This LCD supports integrating evidence-based glucose monitoring, including HbA1c and blood glucose testing, into home health plans of care for Medicare beneficiaries with Type II diabetes. Skilled nursing may administer daily insulin only when beneficiaries are physically or mentally unable to self-inject and no one else can, and plans of care must document functional impairments; HbA1c testing is recommended quarterly for therapy changes or uncontrolled patients and at least twice yearly for those with two consecutive results meeting plan-of-care goals.
