Nerve blocks and electrostimulation/electromagnetic stimulation for treatment of multiple neuropathies or peripheral neuropathies caused by underlying systemic diseases are considered investigational or not medically necessary and are not covered; ultrasound guidance for these non-covered injections is also denied. The policy provides no coverage criteria, documentation requirements, or frequency limits for other uses of nerve blocks; applicability to focal or non-systemic neuropathies is not addressed and may require manual review.
