Autonomic function testing is covered when used diagnostically to evaluate symptoms or clinical changes suggestive of autonomic dysfunction (e.g., autonomic neuropathy, POTS, vasodepressor syncope, distal small fiber neuropathy, or recurrent unexplained syncope) and when results will inform clinical decision-making. Routine screening (including routine diabetic screening), testing solely for monitoring diabetes/hepatic/renal disease, investigational devices (e.g., ANSAR ANX 3.0 or ESC-only devices), duplicative combined tests without required beat-to-beat recording, and equipment lacking required monitors are not covered; documentation of symptoms, exclusion of common causes, equipment capabilities, and provider qualifications must be maintained.
