Non-invasive peripheral venous vascular and hemodialysis access studies are covered when signs or symptoms of altered venous blood flow are present, the test results are necessary for management or surgical planning, and the test is not redundant. Specific covered indications include suspected DVT or PE in candidates for anticoagulation or invasive treatment, chronic venous insufficiency with ulceration or symptomatic varicosities, venous mapping prior to dialysis fistula or when prior vein procedures/conditions or obesity obscure vein assessment, and Doppler studies of compromised hemodialysis access; tests must be ordered and results used by the treating physician and performed/interpreted by credentialed personnel or accredited labs.
