Electrodiagnostic testing (NCS and EMG) is covered when clinically indicated for focal/compressive neuropathies, traumatic nerve injury, generalized neuropathies, neuromuscular junction disorders, radiculopathy, myopathies, plexopathies, and for localization prior to targeted injections. Coverage requires documented clinical history, real-time acquisition and interpretation by appropriately trained/credentialed providers; NCS performed alone is generally not covered except for a limited NC-stat® single-arm use in high pre-test probability carpal tunnel syndrome. Multiple billing and device limitations apply: NCS is payable once per named nerve, preconfigured-array codes once per limb, sNCT and certain portable/automated devices or delayed/telemetry-only interpretations are non-covered, and routine monitoring for diabetic or ESRD polyneuropathy is not covered.
