Flow cytometry is covered for diagnosing, classifying, staging or monitoring hematologic and selected non‑hematologic conditions (eg, MDS/CMPD/leukemia, lymphomas, plasma cell disorders, PNH, MRD, HIV CD4 counts, transplant monitoring, mast cell and platelet disorders, select DNA ploidy once for localized carcinomas, molar pregnancy, and evaluation of blood/body fluids/CSF/bone marrow) and is excluded for routine physicals and indiscriminate “screening” panels. Coverage requires documentation of the specific suspected diagnosis, clinical/morphologic findings, quantitative cell counts and available radiology/cytogenetics, that the lab selected an appropriate marker panel, written justification for any >24 markers (with marker‑by‑marker rationale and full final report data for redeterminations), and limits HIV monitoring to no more than every 3 months in stable patients.
