Medicare covers bed pans (E0275, E0276) for beneficiaries who are bed-confined and otherwise requires that items fall within a Medicare benefit category and be reasonable and necessary. Multiple bowel management-related items (rectal inserts A4337, electrical continence aids, fecal collection systems, gravity enema A4458, pulsed irrigation E0350/E0352, incontinence garments A4520, and underpads A4553/A4554) are specifically denied or statutorily non-covered. Claims require a Standard Written Order/WOPD before billing or delivery, adherence to HCPCS coding guidelines, and maintained proof-of-delivery and other documentation per LCD-related articles and the Supplier Manual.
