Electrodiagnostic testing (NCS and EMG) is covered when performed by appropriately trained/credentialed clinicians and when pre-test clinical history and exam clearly document the need; both modalities are usually required together for peripheral nervous system diagnoses. Routine testing for diabetic or ESRD polyneuropathy, Current Perception Threshold/sNCT, testing with non–real-time or handheld discriminatory devices, and certain EMG techniques/usages (e.g., early post‑trauma EMG, exclusive intrinsic foot muscle testing for proximal lesions, multiple EMGs at the same site for botulinum optimization) are not covered or not established as reasonable.
