Covered only for men with symptomatic androgen deficiency confirmed by laboratory testing; therapy is excluded for asymptomatic or age‑related low testosterone and is contraindicated in men with breast or untreated prostate cancer or those desiring fertility. Key requirements: at least two separate morning serum testosterone measurements (or two free/bioavailable), plus LH or FSH (if LH/FSH low evaluate pituitary including prolactin and chronic disease), documented symptoms and comprehensive exam to exclude other causes, documented discussion of FDA‑listed risks (thromboembolism, erythrocythemia, hypertension), use the lowest dose to achieve low‑normal testosterone (topical preferred), and complete chart documentation (baseline/monitoring PSA if performed).
