Medicare covers CTP application for chronic non-healing lower extremity wounds (primarily diabetic foot ulcers and venous leg ulcers) that have failed appropriate conservative care (generally >4 weeks), provided wounds are partial- or full-thickness with a clean granular base, ≥1.0 cm², free of active infection/osteomyelitis, and have adequate perfusion (e.g., ABI ≥0.60 or toe pressure >30 mmHg). Coverage requires documentation of prior conservative therapy, wound measurements, systemic disease management, and adherence to product labeling; limits include a maximum of 10 applications per wound per 12-week period, typical treatment not to exceed 12 weeks, and exclusion for specific contraindications (uncontrolled systemic disease, active infection, allergy, continued smoking without cessation efforts).
