Vitamin D Assay Testing

Medically necessary vitamin D testing (primarily 25‑OH Vitamin D) is covered for specified clinical conditions such as CKD stage III+, liver disease, calcium metabolism disorders, malabsorption, rickets/osteomalacia, osteoporosis under defined clinical criteria, and monitoring replacement therapy related to listed conditions. Measurement of 1,25‑OH Vitamin D is limited to specific indications (e.g., unexplained hypercalcemia/hypercalciuria with suspected granulomatous disease or lymphoma, genetic childhood rickets, tumor‑induced osteomalacia, nephrolithiasis). Routine population screening is not covered, both assays need not be done for every case, and repeat testing is generally restricted—repeat testing between 20–50 ng/ml is usually unnecessary unless documented, while levels ≥60 ng/ml may be retested until normalized.
