GlycoMark® Testing for Glycemic Control

This policy states that the GlycoMark® assay (1,5-anhydroglucitol, 1,5-AG) is not a covered test for monitoring glycemic control. All uses of this assay are non-covered under this policy.

"The GlycoMark® assay (1,5-anhydroglucitol, 1,5-AG) is not covered for glycemic control monitoring (non-covered test)."
