Visual electrophysiology testing (VEP/VER and ERG, including mfERG) is covered when used to evaluate and distinguish retinal versus optic nerve disease, confirm MS when clinical criteria are inconclusive, assess a broad range of retinal disorders (toxic, ischemic, autoimmune, hereditary, traumatic), and for intraoperative optic nerve monitoring. Use of VEP/ERG for glaucoma diagnosis or management is considered investigational and not covered. Testing must be performed by physicians with documented accredited training and expertise, and mfERG for chloroquine/hydroxychloroquine toxicity should follow AAO guidelines.
