Peripheral nerve blocks are covered for specified diagnostic and short-term therapeutic indications (e.g., mononeuritis evaluation, CRPS after nerve injury, occipital/trigeminal/suprascapular nerve diagnoses, Morton’s neuroma, and preemptive or continuous postoperative analgesia). Coverage is limited to short-term use (generally up to three injections per anatomic site in six months, no more than two sites per session), with diabetic and systemic-disease-related peripheral neuropathies and dry needling considered non-covered/investigational; documentation of diagnosis, prior studies, and response to blocks is required.
