Policy Summary
Noridian covers only evening salivary cortisol testing as medically necessary for diagnosing Cushing’s syndrome and considers all other salivary hormone measurements (e.g., thyroid, testosterone, estrogen, parathyroid, growth hormone) investigational and not covered. Testing must be ordered and used promptly by the treating physician or authorized practitioner per Medicare rules (42 CFR 410.32) with chart documentation of signs/symptoms or clinical rationale for suspected Cushing’s, and exams performed for non-diagnostic purposes are excluded (42 CFR 411.15).
