Visual electrophysiology (VEP/VER and ERG/mfERG) is covered to evaluate visual pathway function, confirm multiple sclerosis when clinical criteria are inconclusive, assess optic nerve diseases (e.g., optic neuritis, ischemic optic neuropathy, toxic/nutritional amblyopia, compressive lesions, injury/atrophy), rule out malingering, and for intraoperative optic nerve monitoring. ERG (including mfERG) is covered for diagnosing retinal dysfunction and retinal degenerations, toxic retinopathies, ischemic and autoimmune retinopathies, post‑trauma assessment, and detection of chloroquine/hydroxychloroquine toxicity per AAO guidance. VEP/ERG use for glaucoma diagnosis or management is considered investigational and not covered; testing must be performed by physicians with documented accredited training and appropriate expertise.
