Medicare covers chiropractic manipulative treatment (CMT) performed by a State‑licensed chiropractor only for manual manipulation of the spine to correct a documented spinal subluxation demonstrated by x‑ray or physical exam, with a direct therapeutic relationship and reasonable expectation of improvement. Maintenance therapy, extraspinal manipulations (CPT 98943), and other diagnostic/therapeutic services furnished by chiropractors are not covered; documentation must specify spinal levels, medical necessity, and use the AT modifier for active/corrective treatment. Contraindications to dynamic thrust (relative and absolute) must be considered and documented, including risk discussions when appropriate.
