Electroretinography (ERG) is covered when used to diagnose retinal dysfunction or distinguish retinal from optic nerve disease, and for a broad set of retinal conditions including toxic retinopathies, diabetic and vascular retinopathies, inherited retinal dystrophies, retinal detachment, and assessment when the fundus cannot be visualized. Multifocal ERG (mfERG) is covered for confirmation of suspected chloroquine/hydroxychloroquine toxicity per AAO guidance but is not recommended for routine primary screening; use of any ERG modality for glaucoma diagnosis or management is investigational and non-covered. Testing must be performed under general supervision and interpreted by a qualified physician, and claims must follow the Local Coverage Article A56672 and applicable CMS rules.
