Electroretinography (full-field ERG and mfERG) is covered to evaluate retinal function and to distinguish retinal versus optic nerve pathology, including a range of toxic, vascular, autoimmune, hereditary, and traumatic retinal conditions; mfERG is also covered to confirm suspected chloroquine/hydroxychloroquine toxicity per AAO guidance. Use of ERG (all forms including PERG and mfERG) for glaucoma diagnosis or management is considered experimental/investigational and is not covered. Testing must be performed under general supervision and interpreted by a qualified physician, and claims must include appropriate ICD-10 diagnosis codes supporting medical necessity.
