MIGS devices are covered for adults: one trabecular aqueous stent per eye is covered when placed at the time of cataract surgery for mild-to-moderate open-angle glaucoma while the patient is on ocular hypotensive medication. One subconjunctival or trabecular stent per eye is covered as a standalone procedure for refractory glaucoma when there is prior failure of filtering/cilioablative surgery or uncontrolled IOP (progressive damage or mean diurnal medicated IOP ≥20 mmHg) on maximally tolerated medical therapy; provider training/credentialing and documentation of indications are required. Concurrent MIGS with an aqueous shunt or multiple MIGS procedures at the same time (including certain combinations with phaco) are non-covered.
