Micro-Invasive Glaucoma Surgery (MIGS)

MIGS is covered for adults with mild-to-moderate open-angle glaucoma when a single trabecular aqueous stent is implanted per eye at the time of cataract surgery while the patient is on ocular hypotensive medication. Standalone subconjunctival or trabecular stents are covered for refractory glaucoma after prior filtering/cilioablative procedure failure or for uncontrolled IOP (mean diurnal medicated IOP >=20 mmHg) on maximally tolerated medical therapy (typically >=4 medication classes or documented intolerance). MIGS is not first-line for mild-moderate disease, multiple MIGS procedures or concurrent MIGS plus an aqueous shunt in the same eye at the same time are not covered, and providers must be appropriately credentialed (XEN45 requires specific trabeculectomy/bleb experience).
