Policy Summary
Spinal cord stimulation is covered primarily for chronic intractable neuropathic pain and, with more limited value, for severe intractable nociceptive pain after failure of conservative therapies. Coverage requires multidisciplinary evaluation including psychological and physical assessments, a documented percutaneous trial demonstrating at least 50% pain or analgesic reduction plus functional improvement, and permanent implants must be performed in an ASC or hospital with specified documentation available for review.
