Supervised Exercise Therapy for the Treatment of Peripheral Arterial Disease with Symptomatic Lower Extremity Intermittent Claudication

Medically supervised exercise therapy is covered for beneficiaries with lower-extremity peripheral arterial disease who have symptomatic intermittent claudication and documented activity limitations attributable to the claudication, provided there is a physician referral, a signed plan of care, and appropriate documentation that the patient can tolerate exercise. Sessions must occur in a hospital outpatient setting or physician office under direct supervision, generally be 30–60 minutes and at least three times weekly, with initial coverage up to 36 sessions over 12 weeks and a possible additional 36 sessions after a new referral if documentation requirements are re-met.
