Corneal Hysteresis

Corneal hysteresis (CH) assessments are not covered when used for risk assessment or for monitoring progression of ophthalmic disease activity. No indications, documentation requirements, or frequency limits are specified in this policy beyond the blanket non-coverage for those uses.
