Hypoglossal nerve stimulation (HNS) is covered for adults aged ≥22 with BMI <35 and moderate‑to‑severe OSA (AHI 15–65) who have predominantly obstructive events and documented CPAP failure or intolerance despite optimizations, provided DISE shows absence of complete concentric palatal collapse and no anatomic contraindications. Coverage requires use of an FDA‑approved HNS device, documented shared decision‑making, objective CPAP data, appropriate surgeon and sleep‑physician training/certification, and procedural/facility accreditation; multiple cardiac, pulmonary, neurologic, pregnancy, and device‑compatibility conditions are contraindications.
