Policy Summary
Medicare coverage disallows duplicative germline DNA/RNA testing of the same genetic content in the same beneficiary; initial testing of previously untested germline content is considered reasonable and necessary. Gene panels that include previously tested content may be covered only if the remaining non-duplicative components have established clinical utility, and any one specific germline test is limited to one unit of service per lifetime; providers must document prior testing status and justification when applicable.
