CT perfusion (CTP) with automated post-processing is covered for patients with acute ischemic stroke caused by unilateral proximal anterior-circulation large vessel occlusion when evaluated at a stroke center to aid selection for endovascular thrombectomy if trial-based time windows and pre-CTP inclusion criteria are met (DAWN: 6–24 h, NIHSS ≥10, prestroke mRS ≤1; DEFUSE 3: 6–16 h, NIHSS ≥6, prestroke mRS ≤2). CTP is contraindicated in patients unable to receive iodinated contrast (severe renal failure or refractory iodine allergy) and is not appropriate when technical or physiologic factors prevent reliable perfusion measurement or when qualifying LVO/time-window criteria are not met.
